One of the best ways to protect yourself from getting seriously ill from COVID-19 is to get vaccinated. CentraState offers the Pfizer bivalent booster for eligible individuals ages 12+ at our off-site clinics. No registration is required, but please bring your insurance card, CDC vaccine card, and fill out these forms prior to arrival:
